Consent Form Question Type
You can now add a Consent question to a survey directly from the Add Question button in the campaign builder. A consent question pairs editable question text with an uploaded PDF and two customizable response buttons, defaulting to "Accept" and "Decline".
Respondents see the question text, the consent document, and both buttons beneath it. Declining takes them to a page explaining that they have declined the survey and will not be compensated. Anyone who declines by mistake sees a button that returns them to the consent form and clears the declined flag, with no need for support to intervene.
